    作者敘述小時候,人們茶餘飯後常圍繞著廚房的餐桌講述各人的故事,既可以打發時間,同時也是一種傳遞智慧的方法,而這也是書名原文《Kitchen Table Wisdom》的由來。每個人就是一個故事,每一個故事都是親身體驗的,都是真實經歷的。我們從別人的故事所獲得的,或許是對人性的感動、或許是深層的啟示、或許是更大的智慧。

    其中一篇小故事是關於傾聽的藝術,讓我們一同來學習吧!有一位病人當她向別人訴說自己的遭遇時,常遭對方打斷,對方反而開始敘述自己類似的遭遇,病人心中的痛不但不能得到舒解,還要聽別人的痛苦,久而久之,她不再向人講述自己的故事了。作者說「建立人際之間的連繫,最基本且最有效的方法,我想大概就是傾聽了」而且「光是聽就行了」。只要我們發自內心專注的、靜靜的聽對方訴說他的遭遇,對方必能感受到你對他的關懷,這樣「比任何想說的話更具治療和連繫的效果」。

        I just read several your articles on CCHERE, and looks like you have lots of experience in both Western medicine and traditional Chinese medicine. Could you compare them for treating invasive breast cancer? One of my best friends just found she's had aggressive breast cancer, and the tumor is too large to do operation. Now doctors have started chemo therapy on her, and it looks like she's OK in terms of side effects. Still, I want to know if there is anything can be done in traditional Chinese medicine, and what are treatment guidelines in Chinese medicine for treating BC.

          I won't recommend the TCM therapy now because there is no solid evidence that can prove the sole TCM treatment could control the BC. However, emerging researches do indicate the TCM antagonize some side effects of chemotherapies such as hematologic and gastrointestinal effects and nephrotoxicity. Some other side effects, such as the fatal allergy and the heart failure caused by paclitaxel and ADM can hardly be helped by TCM alone.

          I don't know what chemo-medicine your friend has been given, but I am glad she is quite resistant to whatever side effects so far. As long as she can hold, she should stick to the chemo. Certainly, TCM could be a nice back-up when the recurrence occurs. Recurrence, unfortunately, is quite usual to most of the patients, and at this stage, the side effects are more frequently seen and are harder to be overcomed.The TCM would turn out to be the ace in the hole at that time.

          One thing I also want to remind is that you need to make sure the doctor has done the chemo-therapy sensitivity test. The purpose of the test is to select the most sensitive medicine to the BC of your friend.

          Be optimistic and be happy. Optimism sometimes does make the difference.

            As I know, doctors treat her with AC (Adriamycin + Cytoxan) 3-week cycle, and plan to do that for 4 cycles before deciding the next treatment. I don’t think the chemo-sensitivity test has been done. I checked on-line resource and found there are debates whether in-vitro chemo-sensitivity test is helpful or not, and looks like most insurance plans won't cover that test. However, I'd push her to ask her doctors to do that kind of test.

            We pray together and trust God's blessing on her.

              usually,CAF also incudes 5-Fu.(CTX+ADM+5-Fu).

              Of these, EPI is very similar to ADM. But it has lesser side effects

              It seemed to me paclitaxel could be under her doc's consideration too.

              I pesonally do believe the pretest is necessary. As far as I know, it is quite popular in china. But certainly, you need to trust her doctor. After all, I did not specialize in this field.

                One more question to bother you: I'm looking for the information about paclitaxel (Taxol) on pregnant women. My friend is in her 4th (5th?) month of first pregnancy. Doctor may use Taxol + Hercptin (Her/2 receptor positive) on her, depend on if AC is working or not. Looks like very few people have done that.

                Any idea?

                  she is preganant.did her doc ask her to stop the pregnancy?

                  Taxolis is very commonly used. actually, it has been the first pick for the chemo of the BC

                  To be the honest, I don't think it is a good idea to keep her pregnancy at this time, no matter for her or for the fetus' good.

                  But again, please consult with her OB and the doc in charge of her chemo.

                    What I knew is that, none of OBs, surgeons, or oncologists suggested to stop pregnancy. In fact, lots of recent studies showed that the treatment for cancer and pregnancy could be done at the same time. Also, there is no evidence that pregnancy can make BC worse. Thus, she will continue her pregnancy.

                    Let God help her and her baby…

                      again, it is a choice about life. Nobody should try to make her to change her mind as long as she have understood what she is doing.

                      As a retired physician, I cautiously doubt that her docs have said so. They must have told her the potential risks, but certainly, they must also have told her she could have some chance to have a healthy baby. Normally,no guarantee would be out from their mouths b/c they had been trained to be the politicians for the law suit reasons

                      Anyways, I wish the best things would happen to her.

                      come on, take it easy. I am not arguing with you. I wish this is ture too.

                      The pregancy would burden on her health, and the side effects of the chemo would bring the risks to the fetus. Please forgive my ignorance if my common sense in this field was too outdated.I just did a quick search for you. I would be more than happy to see any recent researchs could have solved those potential risks listed for sure.

                      I admire her continuance of her pregnancy,and I wish everything went well.It is not an issue of medical science, instead,it is an issuse of philosiphy about life; It is an issue about taking her chance.

                      And she is on her own.

                      And the new born baby would be on his/her own too.
